Rental Agreement |
| Baby's Heart has a minimum 4 month rental policy for all Doppler Rentals. |
| I understand that my credit card will be charged a monthly rental fee from the day my fetal doppler is shipped to me and each month thereafter. Charges will be reoccurring each month until the fetal doppler is returned. I understand that If I return my fetal doppler before my four month anniversary of the day it was shipped I will still be charged for the remaining months up to and not over the agreed upon 4 month time period. A 6% sales tax will be applied to South Carolina residents only. Initial charges will be made on the day or the day before the fetal doppler is shipped. Any applicable shipping charges will be charged with the first months rental. |
| When I want to return the Babys Heart Doppler Unit, I will follow all return instructions then contact Babys Heart by email at info@babysheart.com. Once Babys Heart has verified with DHL that my Fetal Doppler is in route back to them - I will not be billed for additional rental fees. Babys Heart reserves the right to charge me if my Doppler is improperly packed like I received it without the original packing materials. |
| Should you become 2 months behind on rental fees, you will be responsible for a one-time fee of $12 in addition to rent owed. If your account goes to collections (which occurs once you are 3 months behind on payments), you will be responsible for the full price of the Doppler ($325 for the Babys Heart Lite Doppler, $425 for the Standard non-digital Doppler and $525 for the Digital Doppler) in addition to any rental fees you have paid, plus the $12 fee and a 30% administration fee. |
| Babys Heart Doppler's will perform to manufacture's specifications during the rental period. Babys Heart will replace any Doppler device which does not work properly during the rental period so long as the unit has not been damaged by misuse, neglect, or accident. |
| I accept full responsibility for the Fetal Heart Rate Doppler while it is in my possession and I agree to be responsible for the replacement at retail value ($325.00 for the Lite Doppler, $425.00 for the Fetal Doppler, and $525.00 for the Digital Doppler) if the unit is lost, stolen, or destroyed. I agree to return the equipment rented in clean and working condition. I assume full liability for repair costs if the unit is damaged by misuse, neglect, or accident while in my care. |
| If we are unable to validate your shipping address or credit card information we will make all attempts to contact you and clarify the issue. If we still are unable to validate these, Babys Heart reserves the right to charge the full price for its fetal dopplers before or after shipment. Upon your return of the Doppler we will refund all portions of this charge minus the month's the Doppler was rented. If at anytime Babys Heart detects fraudulent issues during my rental period such as theft, misuse of identity, transfer of my fetal doppler to another or dishonesty, Babys Heart reserves the right to contact the Authorities, Your Bank, Your Local Authorities and report you to all watch list as well as Credit Bureaus. Refunds will be given once property is returned and inspected.. |
| I hereby agree to release, hold harmless and indemnify Babys Heart LLC. and all of its agents from any and all liability for my use of there fetal doppler devices rented hereunder. |
| This rental agreement has been executed and entered into in the State of South Carolina. In the State of South Carolina, theft of rental property is considered a crime punishable with fines and jail time. |
| Babys Heart has a privacy policy. We will not sell your personal information to anyone. We will not use your personal information for any purpose outside of this rental. |
| I have read and understand this agreement and voluntarily agree to its terms. |
